RESPONSIBILITIES:
- Assist to care for Individuals with Developmental Disabilities who has complex medical challenges
- Ensures medications are administered with zero medication errors
- Experienced in HRST
- Conduct daily patient assessment, documenting vital signs and health status of the Individuals we serve.
- Collaborate with interdisciplinary teams, including physicians, therapists and support staff to achieve optimal patients outcomes.
- Train Direct Support Professional to care for the individuals served by the agency.
- Participates in team training sessions and workshops, enhancing team members knowledge and skills.
- Contribute to decrease in hospital readmissions by ensuring the individuals received appropriate follow up care and support after discharge
- Adhered to medication schedules, ensuring timely administration of medications to individuals
- Respond promptly to emergencies, initiating appropriate actions taken to ensure quality care for our individuals.
- Computer literate and comfortable in scheduling individuals' appointments as needed.
