Licensed Clinical Social worker needed at a School Based Health Center located in Several East Hartford Magnet Schools.
POSITION SUMMARY: The SBHC Social Worker will provide age appropriate counseling to students in need of behavioral health care services. Social Worker will conduct groups within all three schools 2 days a week. The clinician will be responsible for the assessment, diagnosis, treatment and/or referral of children/adolescents presenting or referred to their care for evaluation. The Social Worker will be responsible, but not limited to, the provision of crisis assessments, individual therapy, group therapy, family therapy, treatment planning, case management, team/community collaboration, patient/family education, and triage/discharge planning
REPORTING RELATIONSHIPS: The Social Workers report directly to the Chief Operating Officer of Integrated Health Services Inc.

- Performs assessments, develops care plans, and provides direct clinical services as well as appropriate education t service population.
- Functions as an effective member of multi-disciplinary intra/inter service teams by establishing positive working relationships, providing timely and pertinent communication to team and by using positive communication and negotiation skills.
- Coordinates and assures facilitation of an optimal discharge plan for patients by keeping current on knowledge of community resources and involving patient/family/conservator/guardian/etc. as appropriate.
- Demonstrates commitment to professional career growth and competence by identifying own learning needs, actively participating in performance appraisal by identifying own individual goals, keeping pace with current trends/literature, actively participating in continuing education activities.
- Demonstrates knowledge of managed care concepts in order to support and facilitate cost effective, quality, age-appropriate care. This includes timely and efficient access to services, working with managed care entities to establish age-appropriate plan for service, assessing patient’s level of care needed to assure the least restrictive seeing for needed care and providing appropriate documentation to reflect course of treatment.
- Contributes to the development of the service by being actively involved in service-specific planning and/or development, by recommending, initiating and supporting quality planning, improvement and control strategies, and by responding to change in a productive manner.
- Providing crisis intervention to children/adolescents as needed
- Acts as a referral source to school and community.
- Develops and implements and modifies treatment plans.
- Complete all required documentation in accordance with departmental regulation and DCF mandates.
- Competency to provide sound individual, family and group therapy.
EDUCATION AND EXPERIENCE
Education Master Degree in Social Work, Marriage and Family Counseling, or Mastes in Counseling.
LCSW and LMFT preferred
Job Related Experience Prior experience in BH field, preferably specific to prospective service setting/level of care/age specific population preferred. Strong clinical skills, written and verbal communication skills, knowledge of community-based resources, and a minimum of 3 months’ experience in an outpatient clinic setting with adolescents is required.
· Training in DBT desirable
· Familiarity with Abuse and Neglect reporting mandates desirable
· Knowledge of Community based resources desirable
Experience with pre authorization and billing process desirable
