What will you be doing:
The Intake Coordinator coordinates activities of order intake, evaluation of admission criteria, referral/authorization of cases, and transition of care for the Home Health Agency, tracking of referrals.
What will you come with:
- Minimum of two (2) year of healthcare experience, in home healthcare preferred.
Specialized knowledge
- Knowledge of the Home Health Services
- Knowledge of referral/authorization process
- Knowledge of computer clinical programs such EHR/EMR system(s)
Skills & Abilities
- Great communication skills, both written and spoken.
- Bi-lingual preferred
- Strong organization skills and be detail oriented.
- Ability to maintain confidentiality of sensitive information.
