Integrated Interventions LLC is looking for a Case Manager to join our active and energetic team! Case Managers work on the ground level with students to develop a plan that will help them live a healthy, sustainable lifestyle. Case managers assist in the daily operation of the program providing case management services for 6-10 students. The Program Coordinators are responsible for the successful operation of the students on their caseload to include working alongside and training Mentors to ensure that they are equipped to serve our students.

Integrated Interventions is a premier transitional living program for youth aged 17.5 and older. Our program is nationally recognized as a new spin on the traditional residential treatment model. Our staff work diligently to help students learn the life skills required to be sustainably independent. Students leaving our program go on to live happy, healthy, and productive lives. We are the last step on the therapeutic journey that many of our students have been on for many years.
Essential Job Functions:
- Responding to student and program crisis as part of the 24/7 crisis response requirements
- Adhering to all of the mentoring guidelines, crisis intervention safety, and program expectations.
- Managing assigned students by:
- Meeting with each assigned student 3 times per week for a minimum of 1 hour by in-person meeting, phone call, crisis response, or text message exchange
- Working with the Program Therapists to create and implement treatment plan(s)
- Maintaining and completing all required program documentation for each student
- Conducting move-ins and move-outs from the program and/or to different housing.

- Case Manager Weekly Review Forms
- Manage transportation and student appointments and schedules.
- Developing and managing student's projects and communicating with mentors and students accordingly.
- Complete 30-minute parent phone call 1 time per week per student
- Communicating with all Mentors and Lead Mentors regarding coordinating events, student needs, and expectation of student tasks and projects.
- Managing the Lead Mentors day-to-day operations and training
- Ensuring mentors and students follow the safety requirements during projects.
- Cover mentoring shifts to ensure student coverage and complete on-call weekend assignments as required.
- Work in student homes alongside Mentors as further and ongoing training on program expectations and operations
- Train and discipline Mentors and Lead Mentors as needed.
- Complete designated tasks as assigned by Directors
Minimum Requirements:
- Have a high school diploma or equivalent
- 21 years of age
- One or more years of related experience in the field of case management, transitional housing supports, or working with individuals with developmental disabilities in an employment setting
- Possess a valid driver's license
- Must have a CLEAN and CLEAR driving record
- Must be able to pass a background check, including fingerprinting and drug testing
- Must secure and maintain First aid and CPR certification within 30 days
- Technology savvy – Especially Google Suite, PC, and cellular devices.
- Ability to life up to 50 lbs.
