Enhanced Care Management (ECM) is a comprehensive, patient-centered program designed to provide intensive care coordination and support to individuals with complex medical and social needs. The program aims to improve health outcomes by addressing physical, behavioral, and social determinants of health.
Position Summary :
The Patient Engagement Specialist is responsible for providing support to the ECM program by conducting outreach to initiate enrollment and completing patient enrollments when the case manager is unavailable and assisting with ongoing care coordination. This role works closely with case managers and other healthcare professionals to ensure comprehensive, patient-centered care for individuals. The Patient Engagement Specialist will play a key role in improving patient engagement and facilitating access to ECM services.
Requirements : Qualifications :
- High school diploma or equivalent
- Spanish proficiency
- Experience in patient outreach preferred.
- Strong interpersonal and communication skills with the ability to engage patients from diverse backgrounds.
- Ability to work independently while collaborating effectively within a multidisciplinary team.
- Proficiency in Microsoft Office Suite and experience with electronic health records (EHR) systems preferred.
