Minimum 5 years experience required. NO FIRST TIME TRAVELERS!
Unit is an inpatient 3-5 day length of stay unit that needs a CSW that works independently, is aware of and can utilize community resources to provide patients with discharge follow up recovery and care. Will be expected to facilitate groups, set up transportation, identify resources and support for the patient and ensure that the patient follows through with discharge plan. Staffed with RN's, LPN's, Mental Health Technicians, provider residents, APP's.

- Psychiatric Hospital exp required. Case Management & Short Term Acute Care Inpt Medical Social Worker exp preferred.
