Location: New Haven, CT or Boston, MA Hybrid 3x/week in office
Data Entry role for the Patient Services Team
Receive Patient Services Enrollment Form from HCP via online, fax, phone call or direct mail
Review Enrollment Form and create patient case in Patient Services CRM, Alex iPatient
Input all consents and check for required missing information
Collaborate with team to collect missing information from patient or HCP as needed and update Enrollment Form
Allocate case to appropriate Client Point of Contact based on therapeutic area and zip-to-territory alignment
Assist with additional Data Entry needs
