Job Description:

Days 9a - 5:30p
Acute Hospital Experience Required.
NOTE - This position is for a social worker with acute care medical (not behavioral) social work experience in discharge planning, facilitating video conferences for extended legal mental health holds, transferring mental health patients to inpatient psychiatric facilities, providing letters of hospitalization and complex discharges for patients in need of long-term post-acute care with minimal to no family support or funding (i.e. facilitating conservatorship, state funding, etc.)
Able to cover 20—25 patients on the Medical Surgical units. Adhere to CMS required documentation of Important Message, social service initial and ongoing assessment, Patient Choice, Discharge disposition, Readmission Assessment, and social service referrals within policy. Assist with placement of patients to psychiatric facilities and provide resources. Attend family meetings, work with community agencies for placement. Assist ED Case Manager with psychiatric patients, as indicated. Willingness to work 1 weekend per month.
