Job Title : LPN - Continuing Care
Job Location : Grand Rapids, MI
Job Duration : 3 Months contract
Schedule : Nights (6 : 30 PM - 7 : 00 AM), 12-Hour Shifts, Weekend Rotation (Every Other)
Job Summary :
The Licensed Practical Nurse (LPN) in Continuing Care provides direct patient care under the supervision of a Registered Nurse (RN), Mid-level Provider, or Physician, contributing to the assessment, planning, implementation, and evaluation of patient care. The role involves a collaborative approach with the healthcare team, ensuring patients receive high-quality care in our 165-bed facility, which includes inpatient rehabilitation and long-term care units.
Responsibilities :
- Assist with the assessment and monitoring of patient’s physical, psychological, and emotional needs, reporting any changes to the RN / Mid-level Provider / Physician.
- Provide direct patient care in accordance with care plans, policies, and procedures.
- Educate patients and families about treatments, medications, and equipment in coordination with the RN / Mid-level Provider / Physician.
- Accurately document patient care in both electronic and written records, following guidelines and policies.
- Communicate effectively with patients, families, and healthcare team members to ensure continuity of care.
- Ensure the cleanliness of patient environments and equipment, reporting any malfunctions promptly.
- Participate in quality monitoring activities and advocate for patient privacy and confidentiality.
- Willingness to work as a CNA as needed, and float across all continuing care spaces (Cedar, Kentridge, Leffingwell).
