The Physical Therapist is responsible for assuming care, managing and coordinating patient treatment as directed by the referring physician. The Physical Therapist responsibilities include, but are not limited to, the following :
Completes initial evaluation and treatment of physical therapy patients
Completes set up plan of care and goal for physical therapy after initial evaluation
Documents physical therapy patients in accordance to the guidelines of the Rehab Department policies
Supervises PTA and PT aide
Manages and coordinates patient treatment as directed by the referring physician reports to the Director of Rehabilitation

While performing the duties of this job, the employee is frequently exposed to moving mechanical parts, outside weather conditions, extreme cold, extreme heat, and risk of electrical shock. The employee is occasionally exposed to wet and / or humid conditions and fumes or airborne particles. The noise level in the work environment is usually moderate. Expected exposure to hazardous materials, infectious waste, blood and body fluids is high.

Bachelors degree from program accredited by the American Physical Therapy Association preferred
Current Oklahoma Physical Therapy license OR be eligible for license in the state of Oklahoma AND able to practice under the Oklahoma Physical Therapy Practice Act. Must have completed licensure by 6 months after initial temporary license is issued. (Must present letter / permit proving eligibility upon hire.)
CPR Certification
Experience in Physical Therapy preferred
Must be able to communicate effectively in English
This job requires the incumbents to operate an INTEGRIS-owned vehicle OR personal vehicle (non INTEGRIS-owned) and therefore must have a current Driver's License from the state of residence, driving record which is acceptable to our insurance carrier and auto insurance verification.
