Essential Job Functions
The primary duties of this position include but are not limited to the following. The work is expected to be completed within the assigned schedule, hourly commitment, and budget. Compliance is expected with Pioneer Valley Hospice & Palliative Care's Policies and Procedures, agency and professional standards of practice, ethics and to facilitate the mission of Pioneer Valley Hospice & Palliative Care.
Essential Functions
- Formulate and utilize spiritual assessments, interventions, outcomes and care plans in order to contribute effectively to the well-being of the person receiving care.
- Provide spiritual care that respects diversity and differences including, but not limited to culture, gender, sexual orientation and spiritual / religious practices.
- Review initial spiritual assessment of newly admitted patients and families at weekly IDT meetings, as requested; respond to referrals and requests for services within standards.
- Contact by telephone all referred patients / families / caregivers within five days of admission unless requested sooner by patient / family (with exception of those who clearly decline spiritual support).
- Serve as liaison between patients / families and local clergy as needed and as directed by plan of care.
- Coordinate with local clergy spiritual and religious rituals (prayer, baptism, etc.) as requested for patients / families.
- Ability to represent and articulate Integritus HealthCare mission, vision and strategies in developing, implementing, and coordinating the SC program.
- Other duties as assigned by the Clinical Director.
