Partnership Health Center in Williamsport, PA is seeking a full-time RN Care Coordinator to work on-site at our Lycoming County Partnership Health Center. The position is Monday-Friday and pays competitively. The Integrity Health RN Care Coordinator works in collaboration and continuous partnership with chronically ill or "high-risk" patients, their family / caregiver(s), clinic / hospital / specialty providers and staff, and community resources in a team approach. A rewarding and challenging position, the successful candidate will meet the following criteria :
The Care Coordinator will be expected to engage with between 10-15 patients per workday with engagement defined broadly as :
Face to Face visit
Telephonic engagement
Engagement with Treating Providers
Documentation Research, Analysis, and Care Planning
A typical day for the Care Coordinator will entail spending half of the day conducting extended patient meetings or other engagements as described above (approximately 30-45 minutes long). The other half of the day will be spent on follow-up with patients, family / caregiver(s), providers, and community resources via secure email, phone calls, text messages, and other communications.

Education : R.N. Required.
Graduation from an accredited university or community college with a background in science, including a degree in Biology, Chemistry, Nursing, Anatomy and Physiology, Public Health, Behavioral Science, or a similar degree.
Experience :
5-10 years experience in a variety of settings including Hospital, Ambulatory Care, Post-Acute Care, Health Plan / Benefits Management, and Disease Management
