Integrity Home Care Hospice is looking for compassionate and dedicated Registered Nurses (RN) to join our team in our Joplin, Missouri office. This office services our adult and geriatric clients on a per visit basis in territories throughout Southwest Missouri.

Responsibilities of an Admissions Registered Nurse (RN):
Completes an initial comprehensive assessment of patient and family/caregiver to determine home health needs.
Provides a complete physical assessment and history of current and previous illness(es); Provides professional nursing care,
Initiates the plan of care;
Uses health assessments to determine nursing needs and any follow up needed;
Develops a plan of care that identifies interventions which will allow patient and family/caregivers to meet their goals;
Administers medications and treatments as prescribed by the physician in the physician’s plan of care
Provides education to the patient and family/caregiver as appropriate per assessment and plan of care;
Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given;
Records pain/symptom management changes/outcomes as appropriate;
Documentation will be completed and returned to the office within 24 hours of assessment;
Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required;
Assists the patient and family/caregiver and other team members in providing continuity of care;
Ensures that supplies, medications and other necessary items and ancillary services are available and delivered/provided when needed;
Actively participates in quality assessment performance improvement teams and activities;
Participates in on-call duties as needed;
Be knowledgeable of admission process and paperwork;
Represent the Company in a courteous and professional manner at all times;
Complies with Compliance Plan;
Complies with HIPAA Policies. Maintains confidentiality of information relating to the patient and family. Will discuss only those aspects necessary to the care and treatment of patient and family with those directly involved in the patient’s care;
As business needs may change, may perform additional responsibilities assigned by supervisor that are reasonably related to the position.
Qualifications for a Registered Nurse (RN):
Must be minimum 18 years of age per state guidelines.
Registered Nurse (RN) license required
Home Health experience preferred but not required.
Experience in care management, care coordination, discharge planning, etc. is preferred
