Job Responsibilities
Your primary responsibilities as a Personalized Patient Advocate will include:
- Providing one-on-one nursing care to patients in their homes
- Assessing patient needs and developing individualized care plans
- Collaborating with other healthcare professionals to ensure seamless care coordination
Requirements
- CURRENT LPN LICENSE REQUIRED
- CPR CERTIFICATION (NO ONLINE CLASSES ACCEPTED)
- TB SKIN TEST (CURRENT WITHIN LAST 12 MONTHS)
