Job Responsibilities
- Delivering high-quality nursing care in post-acute care facilities
- Observing residents' conditions and notifying the nursing team
- Monitoring and reporting vital signs
- Maintaining a clean and safe environment through sanitizing surfaces and changing linens
- Assisting with personal hygiene, feeding, grooming, and other activities of daily living (ADLs)
- Repositioning residents and transporting them as needed
- Liaising between staff, residents, and family members for ongoing communication and transparency
- Engaging regularly with residents and responding to their needs
- Adhering to care plans, laws, regulations, and policies
