Job Responsibilities
- Deliver high-quality nursing care in post-acute care facilities
- Maintain resident condition updates for the nursing team
- Monitor and report residents' vital signs
- Ensure a clean and safe environment by sanitizing surfaces and changing linens
- Assist residents with personal hygiene, feeding, grooming, and daily activities
- Reposition and transport residents as needed
- Liaise between staff, residents, and family members for communication and transparency
- Engage with residents regularly and respond to their needs
- Follow and adhere to care plans and applicable laws and policies
Qualifications and Skills
- Current Connecticut Certified Nursing Assistant (CNA) certification
- Understanding of general nursing care procedures and terminology
- Effective communication with medical professionals, patients/residents, and family members
- High attention to detail
- Lifting requirement of at least 50 lbs
