Job Description
- Deliver high-quality nursing care in post-acute care facilities
- Observe and report on resident conditions and vital signs
- Ensure a clean and safe environment by performing tasks such as sanitizing surfaces and changing linens
- Assist residents with personal hygiene, feeding, grooming, and other activities of daily living (ADLs)
- Reposition and transport residents as needed
- Liaise between staff, residents, and family members to support ongoing communication and transparency
- Engage with residents on a regular basis and respond to their needs
- Adhere to care plans and applicable laws, regulations, and policies
Required Skills and Qualifications
- Current Connecticut CNA certification
- Demonstrated knowledge of nursing care procedures and clinical terminology
- Detail-oriented and able to communicate effectively with medical professionals, residents, and family members
- Ability to lift a minimum of 50 lbs
