Job Responsibilities
- Implementing best practice nursing care in post-acute residents
- Monitoring resident conditions and providing updates to the nursing team
- Measuring and documenting basic vital signs
- Providing assistance with grooming, feeding, toileting, and other ADLs
- Promoting a clean and safe environment through hygiene practices and clearing hazards
- Transporting residents and assisting with movement
- Building trusting relationships through daily interactions
- Following and adhering to care plans and applicable rules, regulations, and policies
- Bridging communications between staff, residents, and family members
Required Skills and Qualifications
- Current Connecticut CNA certification
- Understanding of general nursing care procedures
- Ability to communicate with medical professionals and patients/residents
- Attention to detail and ability to lift at least 50 lbs
