Job Description
- Implement evidence-based care practices for post-acute residents
- Monitor resident conditions and document vital signs
- Assist with daily living activities, such as grooming and feeding
- Promote a culture of safety and hygiene within the facility
- Bridge communication between staff, residents, and family members
Required Skills and Qualifications
- Current CNA certification in Connecticut
- Ability to communicate effectively with medical professionals and patients
- Strong attention to detail and organizational skills
- Able to lift at least 50 pounds
