What are the responsibilities and job description for the Discharge Planner position at Intensive Specialty Hospital?
Intensive Specialty Hospital is a Long-Term Acute Care (LTAC) hospital. We provide cardiopulmonary care, ventilator and respiratory care, complex wound care, rehabilitation therapies, addiction medicine program, geriatric psychiatry program, multidisciplinary pain program, and outpatient counseling program. With four locations in Shreveport and Bossier City, we will be the largest provider of LTAC services in Northwest Louisiana.

The Discharge Planner provides discharge planning, supportive counseling, and case management services to patients and their family members / significant others, serves as a member of the multidisciplinary treatment team, organizes, and facilitates family / patient conferences, actively participates in quality data collection, and assists with preparation of monthly performance improvement reports.
QUALIFICATIONS :
- Bachelors Degree in social work or related field, Psychology preferred
- Current RN, LPN license, graduate of an approved nursing / medicine program and / or Utilization Review Certificate
- Three to five years experience as a Social Worker or discharge planner in healthcare, LTACH preferred
- Basic knowledge of computer skills – Microsoft Office (Word, Excel, Powerpoint)
- Must be able to read, write, and speak English
