InterCare is searching for a Full-Time (with flexible scheduling options available) MAT Clinical Support Nurse to join our team rotating between our Holland, Pullman, and Bangor locations! At InterCare, you'll find a rewarding and challenging work environment and a competitive compensation. Work Schedule :
Hours are typically Monday - Friday from 8 : 00 a.m. to 5 : 00 p.m., with occasional late patient activity . Days and work schedule may vary due to business need. FLEXIBLE SHIFTS AVAILABLE!!

Minimum Requirements
Possesses a thorough understanding of theory and practices of SUD programming and MAT management acquired through completion of a SUD training, RN program and practical experience in a professional environment.Current RN license in the State of Michigan.Primary Accountability
Serves as a clinical care team member providing high-quality, holistic nursing to patients with Substance Use Disorder (SUD) through a client-centered approach. Responsible for assisting in developing and implementing the SUD programming at all sites, working in collaboration with other team members to assist patients in meeting their recovery goals.
Description of Primary Duties & Responsibilities
1) Develops individualized patient care plans
Performs or assures completion / documentation of a comprehensive patient assessment : medical, functional, cognitive, affective, psychosocial, nutritional and environmentalWorks in collaboration with the BHP (Behavioral Health Provider) and the Primary Care Provider (PCP) to develop and implement an individualized care plan in a person-centered manner, basing level of care on the American Society of Addiction Medicine (ASAM) CriteriaInitiates evidence-based care planning with the patient / family,Assists the patient in developing realistic self-management goals and the implementation of care plans. Assists in the identification of barriers if goals are not met.Monitors the patient, assessing progress on treatment goals and self-management goals.Monthly, or more frequently if needed, reviews and updates care plans in conjunction with the BHP, peer recovery coach and patient to measure patient success.Implement the systems and processes for patient intake, education and continuous follow up and tracking.Supports site staff in the provision of patient education and relapse prevention support.Responsible for assuring the ongoing monitoring of patient compliance in regards to follow up and tracking.Assures the establishment and ongoing performance of huddles to discuss and review SUD programming clients with the interdisciplinary team.Has a strong working knowledge of eCW and works in collaboration with care team to ensure appropriate documentation.2) Monitors health centers to assure compliance with storage and handling requirements for Vivitrol stock, and makes recommendations that will assist in meeting requirements.
Assures all sites have an emergency plan in place in the event of a power outage.Provides inventory support to the sites.Supports site staff with the administration of Vivitrol, when needed.3) Works collaboratively to develop and sustain community relationships
Assists in establishing referral and coordinating agreementsParticipates in outreach efforts to address additional community need.Serves as a liaison to community partners to include : community mental health, judicial systems, substance abuse treatment agencies and other various agencies.Requirements

Job duties are performed in the typical office environment of the organization, which requires ordinary ambulatory skills sufficient to visit other locations.
