What are the responsibilities and job description for the Home Visiting Nurse Practitioner Part-Time, Beaumont - Port Arthur, TX position at Intercede Health?
The Nurse Practitioner is a vital part of the Intercede Health Transitional Care Program and is responsible for supporting our program by providing patient care in a home based and tele-visit setting.
This is a part-time position. The NP will be assigned a specific group of patients to visit with once every 4-6 weeks. Flexible schedule working 4-8 days a month with potential for additional hours upon NP request (approx. 10-15 patients per month)
The primary location of most of the patients is in the Beaumont - Port Arthur Area. This position requires a lot of driving as patients can be anywhere in the area. You can schedule the patients according to location and we do pay for mileage.
Essential Job Responsibilities:
- Provide patient care in a home setting and through tele-management for assigned caseload;
- Provide education to patients and family members, regarding such topics as medication usage and side effects, illness progression, diet and nutrition, medical adherence protocols and crisis anticipation and prevention;
- Collaborate with provider to develop and implement care plans to prevent unnecessary hospitalizations and to coordinate overall patient care;
- Schedule follow up visits and refer to other specialists, if necessary;
- Obtain, review, record history, exam results and diagnostics, as needed
- Document visits, admission, discharge, patient education, health history, etc. in Electronic Medical Record;
- Prescribe medication and authorize drug refills, as directed;
- Prepare and administer medications as directed by clinic provider;
- Maintain supplies and equipment needed for patient management;
- Accurately and thoroughly log all documentation in appropriate systems; and
- Other duties as assigned.
Knowledge, Skills & Abilities
- Nurse Practitioner (with a specialty in Family/Adult or Geriatrics) certification/licensure is required
- BLS/ CPR Certification;
- Medical Malpractice Insurance Required
- 2 years of applicable home health experience;
- Experience using EMR, (eClinical Works preferred); and
- Reliable automobile, valid driver’s license and minimum state required liability auto insurance (Mileage for travel to scheduled appointments is reimbursed)
