What are the responsibilities and job description for the Older Adult Clinician/Contractor position at Intercommunity Action?

Schedule: Minimum 20 hrs/wk
PURPOSE:
Provide strengths-based, person-centered therapy to older adults within the community and over telehealth as needed. Treatment to include diagnostic assessments; individual, group, and family therapy; and supportive services for a diverse caseload of older adults experiencing a variety of behavioral health issues.
FUNCTIONS:
- Provide client-centered, trauma-informed, mobile mental health treatment.
- Enter intake and therapy appointments into EMR at least one week in advance.
- Maintain accurate records of performance metrics and time spent including minimum billing expectations commensurate with scheduled hours. This includes entering progress notes within 3 days of service, including supportive documentation (e.g., treatment plans, safety plans, case management notes, briefs, triage notes, discharge summaries, no show or cancellation notes).
- With the individual’s consent, make a concerted effort to engage identified support system including family and significant others, as it pertains to identified treatment goals.
- Provide an initial and ongoing assessment of presenting issues, diagnostic impressions, and recommended course of treatment.
- Create treatment plan and routinely monitor progress in collaboration with the individual.
- Provide linkage identified resources as needed. This may include coordination with Care Coordinator to facilitate referrals for expanded mental health programming including IOP, D&A, BCM, Psychiatry or community services.
- Coordinate care with other involved professionals (including Senior Community Centers and psychiatric, medical, or social services providers, etc.) to ensure holistic, integrated care.
- Actively participate in staff meetings and provide consultation/support to team members.
- Create a safety plan and provide crisis intervention as needed. This will include follow up and coordination of care when individuals go to the CRC or another acute level of care.
- Maintain required records and documentation, including intake assessments, progress notes, treatment plans, and discharge summaries in compliance with Agency, State, and County regulations.
- Assist with community outreach, education and prevention programs as needed.
- Complete all mandatory trainings (in first 90 days and then as needed) to maintain appropriate credentialing; obtains EBP training as requested by supervisor.
- Function in compliance with agency policies/procedures and federal, state and local laws.
- Attend regular supervision with Older Adult Outpatient Supervisor following supervision policy. Come to supervision prepared to discuss current caseload, weekly productivity and any relevant clinical or administrative questions or concerns.
- Actively participate in appropriate EBP consultation group(s), each held for one hour/month.
- Embrace Sanctuary Meeting and behave in a manner that promotes safety, equity, anti-ageist beliefs, and trauma-informed practices.
- Attend internal and external meetings as requested by supervisor.
- Manage own tax and financial affairs as a contractor.
- Other duties as assigned.
PAYMENT:
- Agency will provide office space and support services including clinical supervision, administrative assistance, phone and computer, IT support, office supplies, and access to RELIAS for training and professional development.
- Contractor will be paid an agreed upon rate per hour for billable services provided.
- Contractor will submit invoices for billable services by the end of business day on Mondays. All required paperwork (including progress notes and up-to-date treatment plans must be completed) before the program supervisor will sign-off on invoices. Services delivered without a corresponding progress note or current treatment plan may not be paid. Late invoices (30 days after service delivery) must be approved by the program administrator.
QUALIFICATIONS:
1. Master’s or Doctoral degree in Human service field.
2. Must be clinically licensed or licensed-eligible in the Commonwealth of PA. If license eligible, will obtain clinical license in no more than 2 years.
3. At least 1 year of experience providing behavioral health therapy to older adults including a demonstrated knowledge of the specific nuances of treating this population.
ADDITIONAL REQUIREMENTS:
1. Required to show proof of Liability Insurance.
2. Must have Valid Childline, PA Criminal History and FBI Clearances prior to seeing clients.
3. Previous training in EBPs or a willingness to obtain training.
- Reliable means of transportation and valid driver’s license from state of residence with a minimum of one (1) year driving experience and a driving record free of at-fault accidents or moving violations for the past year.
- Ability to work collaboratively within a team-oriented environment.
