We are currently seeking a Licensed Medical Social Worker (LMSW) to join our team for per diem home health visits. This flexible, per diem position offers a competitive $85 per visit rate, allowing you to set your own schedule while impacting lives in your local community.

Key Responsibilities for the Licensed Medical Social Worker LMSW:

• Conduct social work assessments and develop personalized care plans for home health patients.

• Provide counseling and support to patients and families to help them adjust to illness, injury, or the challenges of aging.

• Assist patients in connecting with community resources, services, and support networks.

• Collaborate closely with the interdisciplinary healthcare team, including nurses, therapists, and physicians, to ensure holistic patient care.

• Offer crisis intervention, advocacy, and guidance to patients and their families as needed.

• Maintain thorough and timely documentation of services provided in compliance with healthcare regulations.

Minimum Qualifications for the Licensed Medical Social Worker LMSW:

• Required: Current MSW license in the state of Pennsylvania.

• Prior experience in home health, hospice, or a similar healthcare setting preferred.

• Knowledge of local community resources and support services in Lackawanna, Wyoming, Wayne, and Susquehanna counties.

• Strong communication skills and the ability to work both independently and within a team.

• Reliable transportation and a valid driver’s license.

• CPR and First Aid certification preferred but not required.
