- Provide home-based social services to patients dealing with depression, anxiety, poor nutrition or a lack of resources that make managing their injury or disease difficult
- Work as part of a home health team which may include an RN, LVN, CNA, HHA, PT, OT or SLP, focused on the patient's plan of care and individual goals
- Monitor a patient's psycho-social condition and identify social and emotional needs
- Conduct patient assessments, document progress and ensure patient is moving toward goals
- Provide counseling, community resource planning, crisis intervention and advocacy
- Consult with family and caregivers on patient's plan of care and how to help them progressA few must-haves for Home Health / Hospice Medical Social Workers :
- Master's degree in Social Work and active MSW license in CA
- Minimum of 1 years of social work experience preferred, ideally in home healthcare
- Knowledge of state and federal home health regulations
