As a Home Health Licensed Vocational Nurse, here’s a big-picture view of what you’ll do:
- Provide one-on-one, home-based care for an adult patient, including ventilator (vent), tracheostomy (trach) management, and medication administration
- Monitor vital signs, assess patient conditions, and document findings accurately in patient records
- Maintain open communication with the RN supervisor, care team, and family members to ensure continuity of care
- Assist the RN in carrying out the patient’s plan of care, including therapeutic interventions and rehabilitation exercises as prescribed
- Educate the patient and family on the care plan, medication administration, and home safety measures
- Follow healthcare regulations, infection control protocols, and ensure proper maintenance of medical equipment
A few must-haves for Home Health Licensed Vocational Nurses:
- Graduate of an accredited vocational nursing program and active LVN license in CA
- Minimum of 1 year of LVN experience, ideally in home healthcare, skilled nursing, or an acute care setting
- Vent and Trach experience preferred, training available
- CPR certification
- Knowledge of state and federal home health regulations
- Clinical proficiency in nursing, strong communication skills and compassion
