Job Responsibilities
- Provide personalized patient care, working closely with physicians and collaborating with a team of LPNs, CNAs, and aides
- Conduct thorough assessments, document progress, and report changes to physicians
- Educate patients on their plan of care, medication, therapy, diet, and exercise
- Cook family members on home safety and support them in caring for their loved ones
- Ensure goals are met and coordinate discharge from services

Requirements and Qualifications
- Active, unrestricted Registered Nurse (RN) license in the State of Michigan
- At least one year of clinical experience as a Registered Nurse
- Current CPR Certification and reliable transportation
