Job Responsibilities
- Provide home-based social services to patients facing end-of-life issues, including care management, pain, symptoms associated with chronic and terminal illnesses, depression, anxiety, poor nutrition, or a lack of resources.
- Collaborate with the interdisciplinary team, including RNs, LPNs, CNAs, Spiritual Counselors, Volunteers, Hospice Aides, PTs, OTs, or SLPs, to develop and implement comprehensive care plans.
- Assess and address social and emotional needs of patients and families, including bereavement support and counseling.
- Document patient progress and outcomes, ensuring accurate and timely reporting of care provided.
Requirements
- Graduate of a Bachelor's Degree program in social work accredited by the Council on Social Work Education.
- Licensed to practice as a Social Worker in the state in which employed. The license must be active and in good standing.
- Preferred qualifications include hospice experience, palliative care training, or certifications in gerontology or oncology social work.
