Key Responsibilities:
- Delivering high-quality patient care in a home setting
- Working collaboratively with a multidisciplinary team
- Documenting patient progress and reporting changes to the RN supervisor
- Assisting the RN in developing and implementing patient plans of care
- Teaching patients and families self-care techniques and educating on patient care and medication
- Collaborating with the interdisciplinary team to coordinate patient care and plan for discharge
Salary : $55,000 - $65,000
