Nurse Responsibilities:
- Provide high-quality home-based care to patients with illnesses, injuries, and chronic diseases under the supervision of an RN
- Collaborate with a multidisciplinary team to deliver seamless patient care
- Conduct thorough assessments, document progress, and report changes to the RN supervisor
- Support the RN in implementing patient plans of care and teaching self-care techniques
- Educate family members on patient plans of care, medication management, and home safety
- Prioritize effective communication with the interdisciplinary team to coordinate patient care and plan for discharge
Must-Have Qualifications:
- Licensed Practical Nurse (LPN) with an active, unrestricted license in Michigan
- At least one year of clinical experience as an LPN
- Current CPR certification
- Reliable transportation with a valid state driver's license and current car insurance
