**Job Summary:** We are seeking an experienced RN or LPN to join our Hospice division, working collaboratively with interdisciplinary teams to develop and implement personalized care plans. As a key member of our team, you will provide emotional support, pain management, and symptom relief to patients and their families.
Responsibilities:
- Conduct ongoing assessments of patients' physical and psycho-social status
- Develop and implement comprehensive care plans in collaboration with IDGs, patients, and attending physicians
- Provide education and interventions to manage patients' pain and symptoms
- Maintain accurate and timely documentation of patient services provided
Requirements:
- RN/LPN with active, unrestricted license in Michigan
- At least one year of relevant experience as an RN or LPN
- Preferred: one year of management experience, home health experience, and community health experience
