Job Responsibilities
- Provide home-based speech therapy to patients experiencing communication, swallowing, or cognitive difficulties due to illness or injury.
- Collaborate with a multidisciplinary team, including RNs, LPNs, CNAs, HHAs, PTs, and OTs, to develop and implement individualized patient plans.
- Conduct thorough assessments, document progress, and ensure patients meet established goals.
- Educate patients and families on therapy goals, safety practices, and self-care techniques.
