Job Responsibilities:
- Provide in-home care to patients with various medical conditions under the supervision of an RN
- Collaborate with healthcare teams, including physicians, RNs, CNAs, and aides, to deliver quality patient care
- Monitor patient progress, document changes, and report to the RN supervisor
- Assist RNs in implementing patient care plans and teaching self-care techniques
- Educate patients' families on care plans, medication, and home safety
- Participate in interdisciplinary teams to coordinate patient care and plan for discharge
Salary : $55,000 - $65,000
