Your Responsibilities:
- Deliver high-quality home-based care to patients with complex medical needs
- Collaborate with a multidisciplinary team to coordinate patient care and services
- Conduct thorough assessments, document progress, and report changes to the RN supervisor
- Develop and implement individualized patient plans of care, taking into account their unique needs and goals
- Educate family members on patient care, medication management, and home safety
Qualifications:
- Licensed Practical Nurse (LPN) with an active, unrestricted license in Michigan
- At least one year of clinical experience as an LPN
- Current CPR certification
- Reliable transportation with a valid state driver's license and current car insurance
