Job Description:
As a Home Health RN, you'll provide home-based care to patients with illnesses, injuries and chronic diseases—often after discharge from a hospital, rehab or nursing facility. You'll work with a team of physicians and oversee LPNs, CNAs and aides providing patient care. Key responsibilities include examining patients, doing assessments, documenting progress and reporting changes to their physician.
