We are seeking a highly skilled and compassionate Physical Therapist Assistant to join our team in providing top-notch home care services. As a key member of our interdisciplinary healthcare team, you will be responsible for assisting physical therapists in assessing patient needs, developing and implementing treatment plans, and educating patients and their families on proper techniques and exercises.
Job Summary:
- Assist physical therapists in conducting ongoing assessments and creating treatment plans tailored to individual patient needs
- Provide direct patient care, including exercises and therapies as directed by the treating physical therapist
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
- Maintain accurate and timely documentation of patient progress and treatment interventions
Required Skills and Qualifications:
- Current licensure as a physical therapist assistant in the state of Michigan
- At least 2 years of experience as a physical therapist assistant in the last 5 years
- Active CPR certification
- Prior home health experience is a plus
