Job Description
- Job Responsibilities:
- Provide high-quality home-based care to patients with various conditions, often after discharge from a hospital or rehabilitation center.
- Work collaboratively with physicians and oversee LPNs, CNAs, and aides to ensure seamless patient care.
- Evaluate patients, assess their needs, document progress, and report changes to their physician.
- Education patients on their treatment plan, medication, therapy, diet, and exercise.
- Coach family members on the patient's care plan, medication, and home safety.
- Ensure goals are met and coordinate discharge from services.
Required Skills and Qualifications
To succeed in this role, you must possess:
- A registered nurse (RN) license with active, unrestricted status in Michigan.
- At least one year of clinical experience as an RN.
- Current CPR certification.
- Reliable transportation with a valid state driver's license and current car insurance.
