Job Summary
We are seeking a highly skilled Registered Nurse to join our team as a Home Health RN. As a key member of our healthcare team, you will be responsible for providing direct patient care, conducting assessments, developing care plans, and educating patients and their families on disease management and self-care techniques.
Responsibilities
- Conduct thorough assessments of patients' physical and emotional needs
- Develop and implement individualized care plans that address patients' goals and objectives
- Provide high-quality patient education on disease management, medication regimens, and self-care practices

Requirements
- Degree in Nursing (BSN or ADN)
- Active RN licensure in Michigan
- Minimum 1 year of experience in home healthcare or a related field
- CPR certification
- Reliable transportation and valid driver's license
