What are the responsibilities and job description for the Home Health Social Worker position at Interim HealthCare Home Health and Hospice?
At Interim HealthCare, we are seeking a compassionate and dedicated Home Health Social Worker to join our team. As a Home Health Social Worker, you will have the opportunity to make a meaningful difference in the lives of patients and families facing end-of-life care challenges.
Your primary responsibility will be to provide home-based social services to patients dealing with chronic and terminal illnesses, including care management, pain, symptoms, depression, anxiety, poor nutrition, and lack of resources that hinder their journey.
You will work closely with an RN, LPN, CNA, Spiritual Counselor, Volunteers, Hospice Aides, PT, OT, or SLP, as part of a hospice team focused on the patient's plan of care and individual goals. Your role will involve monitoring a patient's psychosocial condition, identifying social and emotional needs, conducting patient assessments, documenting progress, and ensuring patients move toward their goals.
A key aspect of your job will be providing counseling, community resource planning, crisis intervention, and advocacy. You will also consult with family and caregivers on patient plans of care and how to help them progress.
We require our Home Health Social Workers to have a Bachelor's Degree in Social Work from a Council on Social Work Education-accredited program, and be licensed to practice as a Social Worker in the state where employed. While hospice experience is preferred, it is not required.
