About the Role
The Physical Therapist Assistant will play a vital role in our Skilled Home Care division, working closely with our interdisciplinary healthcare team to provide comprehensive care to patients in their homes.
As a key member of the team, the Physical Therapist Assistant will participate in ongoing assessments, develop and implement treatment plans, and provide education and support to patients and their families.
- Participates in ongoing interdisciplinary assessment of the patient
- Acts at the direction of the treating physical therapist and in accordance with the plan of care
- Participates in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s)
- Provides services that are ordered by the physician as indicated in the plan of care
The successful candidate will possess excellent interpersonal skills, be able to work independently, and have a strong understanding of professional standards of practice and payor requirements.
