As a Hospice RN or LPN, your responsibilities will include:
- Conducting ongoing assessments of patients' physical status and psycho-social needs
- Developing and implementing personalized care plans in collaboration with the Interdisciplinary Group, patients, and attending physicians
- Providing education and interventions to manage patients' pain and symptoms, and assessing their response to treatment
- Ensuring access to necessary equipment, supplies, and services
- Regularly attending IDG meetings and revising care plans as needed
Requirements for this role include:
- An active, unrestricted license to practice as an RN or LPN in Michigan
- At least one year of relevant experience as an RN or LPN
- Preferred qualifications include one year of management experience, home health experience, and current CPR certification
