RN/LPN Responsibilities:
- Collaborate with interdisciplinary teams to develop comprehensive care plans tailored to individual patients' needs
- Provide education and interventions to support effective pain management and symptom control
- Maintain accurate and timely documentation of patient services provided
- Attend regular meetings with healthcare professionals to ensure seamless coordination of care
RN/LPN Requirements:
- Active unrestricted license in the State of Michigan (RNs) or current LPN licensure (LPNs)
- Minimum 1 year of relevant experience as an RN or LPN (required)
- Preferred qualifications: 1 year of management experience and community health experience
- CPR certification and reliable transportation required
