Responsibilities:
- Complete ongoing assessments of the patient's physical status and psycho-social status
- Develop a plan of care in collaboration with the Interdisciplinary Group (IDG), the patient/family, and the attending physician
- Provide education and interventions to support effective pain management and other physical symptoms
- Ensure equipment, supplies, and necessary items are available
- Maintain accurate and timely documentation of patient/family services provided
Requirements:
- RN/LPN with active, unrestricted license in Michigan
- At least one year of relevant experience as an RN or LPN
- One year of management experience preferred
- Community health and Medical-Surgical experience preferred
- Reliable transportation with valid driver's license and current car insurance
