Responsibilities
- Provide home-based social services to patients dealing with end-of-life issues, including care management, pain, symptoms associated with chronic and terminal illnesses, depression, anxiety, poor nutrition, or lack of resources complicating their journey
- Collaborate with a hospice team consisting of an RN, LPN, CNA, Spiritual Counselor, Volunteers, Hospice Aides, PT, OT, or SLP, focused on the patient's plan of care and individual goals
- Monitor patients' psychosocial condition and identify social and emotional needs
- Conduct patient assessments, document progress, and ensure patients are moving toward established goals
- Offer counseling, community resource planning, crisis intervention, and advocacy
- Consult with family and caregivers on patient plans of care and how to facilitate progress
Requirements
- Graduate of a Bachelor's Degree program in social work accredited by the Council on Social Work Education
- Licensed to practice as a Social Worker in the state of employment, with an active and good-standing license
- Preferably, experience in hospice care
