Key Responsibilities:
- Deliver patient care in a home setting, working closely with RNs, physicians, and other healthcare professionals
- Take patient vitals, perform assessments, and document progress
- Assist RNs in developing patient care plans and educating patients and families on health-related topics
- Participate in interdisciplinary team meetings to coordinate patient care and plan for discharge
Must-Haves:
- Licensed Practical Nurse (LPN) with active, unrestricted license in the State of Michigan
- One year of clinical experience as a Licensed Practical Nurse (LPN)
- Current CPR Certification
- Reliable transportation and a valid state driver's license
