Home Health Nurse Responsibilities:
- Provide home-based care to patients under the supervision of an RN
- Collaborate with a team of healthcare professionals to deliver quality patient care
- Take patient vitals, conduct assessments, document progress, and report changes to the RN supervisor
- Assist the RN in implementing patient care plans and teaching self-care techniques
- Educate families on patient care, medication, and home safety
- Coordinate patient care with the interdisciplinary team and plan for discharge

Requirements for Home Health Nurses:
- Licensed Practical Nurse (LPN) with an active, unrestricted license in Michigan
- At least one year of clinical experience as a Licensed Practical Nurse (LPN)
- Current CPR Certification
- Reliable transportation with a valid driver's license and current car insurance
