Home Health Nurse Responsibilities:
- Provide high-quality, patient-centered care to individuals with illnesses, injuries, or chronic diseases in their homes under the supervision of an RN.
- Collaborate with a multidisciplinary team of physicians, RNs, CNAs, and aides to deliver comprehensive patient care.
- Take patient vitals, conduct assessments, document progress, and report changes to the RN supervisor.
- Assist the RN in implementing the patient's plan of care and teaching self-care techniques.
- Educate patients and their families on medication management, home safety, and disease prevention.
- Work closely with the interdisciplinary team to coordinate patient care and develop discharge plans.

Requirements for Home Health Nurses:
- Licensed Practical Nurse (LPN) with an active, unrestricted license in the State of Michigan.
- At least one year of clinical experience as a Licensed Practical Nurse (LPN).
- Current CPR Certification.
- Reliable transportation with a valid state driver's license and current car insurance.
