**Home Health Nurse Responsibilities:**
- Provide home-based care to patients with illnesses, injuries, or chronic diseases under the supervision of an RN
- Collaborate with a team of physicians, RNs, CNAs, and aides to deliver high-quality patient care
- Take patient vitals, conduct assessments, document progress, and report changes to the RN supervisor
- Assist the RN in carrying out the patient's plan of care and teaching self-care techniques
- Educate families on the patient's plan of care, medication, and home safety
- Work with the interdisciplinary team to coordinate patient care and plan for discharge

**Requirements for Home Health Nurses:**
- Licensed Practical Nurse (LPN) with an active, unrestricted license in the State of Michigan
- At least one year of clinical experience as a Licensed Practical Nurse (LPN)
- Current CPR Certification
- Reliable transportation with a valid state driver's license and current car insurance
