Home Health Nurse Responsibilities:
- Provide home-based care to patients under RN supervision
- Work with multidisciplinary teams to deliver quality patient care
- Take patient vitals, conduct assessments and document progress
- Assist RN in carrying out patient plans of care and teach self-care techniques
- Educate family members on patient plans, medication and home safety
- Collaborate with interdisciplinary teams for coordinated care and discharge planning

Requirements:
- Active, unrestricted LPN license in Michigan
- At least one year of clinical experience as an LPN
- Current CPR certification
