Job Description
We are seeking skilled Licensed Practical Nurses (LPNs) to join our team as Home Health Nurses. In this role, you will work collaboratively with a multidisciplinary team to deliver personalized patient care. You will take vital signs, conduct assessments, document progress, and collaborate with RNs to develop effective care plans. Additionally, you will teach patients self-care techniques, educate families on medication management and home safety, and facilitate communication with healthcare providers.
